The NOVA Approach to Men's Health

Most men I see at NOVA Wellness don't come in because they read an article about testosterone. They come in because something has been off for a while and they can't explain it — and they've finally decided to stop ignoring it.

The energy isn't there. Training that used to produce results has stopped. The weight has shifted — less muscle, more around the middle — despite nothing obvious changing. The drive and motivation that used to be a given are quieter than they used to be. Recovery takes longer. Sleep is lighter. And somewhere underneath all of it is a low-grade sense of being a diminished version of themselves that's hard to name and harder to bring up.

When they've mentioned it to a doctor, they've usually been told their labs look fine.

That answer is almost never the whole picture. And it's exactly why NOVA Wellness exists.

The Testosterone Conversation Men Aren't Having

Men's testosterone levels have been declining at a population level for decades. The data is consistent across multiple countries and research groups: men today have roughly half the testosterone of men the same age in the 1970s. This is not individual variation. It is a measurable, population-wide shift driven by environmental factors, chronic stress, sedentary lifestyle, sleep disruption, nutritional changes, and endocrine-disrupting compounds that have become ubiquitous in modern life.

What this means practically is that a man in his 40s today may be dealing with the physiological consequences of low testosterone while being told he is completely normal — because he is normal by current population standards. Those standards have shifted significantly downward from what they were a generation ago.

Testosterone begins declining gradually after age 30 — roughly one to two percent per year. By the mid-40s, many men are at levels that produce real, noticeable effects on how they feel and function. The process is gradual enough that most men adapt to it slowly, attributing the changes to stress, work, age, or simply life — rather than to a hormonal shift that can be measured, evaluated, and addressed.

What Low Testosterone Actually Feels Like

The clinical picture of low testosterone in men is rarely dramatic. It doesn't usually announce itself. It accumulates.

The most common presentations I see:

  • Persistent fatigue that isn't explained by how much you slept or how hard you worked. A baseline tiredness that used to feel temporary but has become the default.

  • Muscle loss and body composition changes. Testosterone is one of the primary anabolic signals in the body — it drives muscle growth and maintenance. When it falls, muscle responds less efficiently to training and is lost more easily during caloric restriction. The result is a body that looks and feels different from what training used to produce, without any obvious change in effort.

  • Fat accumulation — particularly in the abdomen. Low testosterone is associated with increased visceral fat storage through multiple mechanisms, including its relationship with insulin sensitivity and cortisol. The belly that appeared in your 40s and won't leave despite reasonable diet and exercise is often metabolic, not purely behavioral.

  • Reduced drive, motivation, and mental sharpness. Testosterone influences dopaminergic pathways in the brain — the systems involved in motivation, reward, and goal-directed behavior. Men with low testosterone often describe a flattening of drive that goes beyond physical — less engagement, less initiative, less of the forward momentum that used to feel natural.

  • Mood changes. Irritability, lower patience, reduced emotional resilience, a shorter fuse — these are common testosterone-associated symptoms that men rarely connect to hormones and that providers rarely ask about in a standard annual visit.

  • Reduced libido and sexual function. Testosterone is the primary driver of sexual desire in men. Its decline often produces a gradual reduction in libido that most men attribute to stress, relationship dynamics, or aging — rather than to a correctable hormonal cause.

  • Slower recovery. From workouts, from illness, from stress. The resilience that used to be a given takes more effort to maintain and bounces back more slowly when depleted.

Why a Standard Panel Misses It

Most men who come to NOVA have had bloodwork done. Most of them have been told their testosterone is normal. And many of them are right — their total testosterone is within the reference range.

But total testosterone is only part of the picture. Here's what a standard panel typically misses:

SHBG — sex hormone-binding globulin. SHBG is the protein that binds to testosterone and renders it biologically unavailable. High SHBG — which rises with age, with certain medications, and with low thyroid function — can leave a man with a normal total testosterone and a genuinely low free testosterone. Free testosterone is what the body can actually use. Two men with identical total testosterone numbers can have dramatically different free testosterone levels depending on their SHBG.

Estradiol. Testosterone converts to estrogen through a process called aromatization. In men, elevated estradiol — which can occur with excess body fat, alcohol use, or without a clear cause — suppresses testosterone production and drives its own set of symptoms including fat accumulation, mood changes, and reduced libido. Estradiol is almost never checked in a standard men's health panel. It should be.

LH and FSH. These pituitary hormones tell us whether low testosterone is originating in the testes or in the brain's signaling to the testes. That distinction changes the clinical approach significantly and is relevant to fertility preservation for men who want to maintain sperm production.

Thyroid panel — full. As we've covered in earlier posts, suboptimal thyroid function produces a clinical picture that overlaps significantly with low testosterone — fatigue, weight gain, cold intolerance, cognitive slowing. Treating testosterone without evaluating the thyroid means potentially treating the wrong variable or addressing only part of the picture.

Fasting insulin and metabolic markers. Insulin resistance and low testosterone are bidirectionally linked — each drives the other. A man with significant metabolic dysfunction may see limited response to testosterone therapy until the metabolic foundation is addressed. We look at the full picture, not just the hormone numbers in isolation.

Testosterone Therapy — What It Actually Involves

When testosterone therapy is appropriate, NOVA uses injectable testosterone cypionate — bioidentical testosterone administered subcutaneously, typically weekly. This is not pellet therapy. It is not a gel that transfers to family members or a patch that causes skin reactions. It is the most pharmacokinetically predictable form of testosterone delivery available, and it is what I use because it is what I can adjust precisely and monitor accurately.

The benefits of appropriately dosed testosterone therapy in men with confirmed deficiency are real and well-documented: improved energy and motivation, increased lean muscle mass, reduction in visceral fat, improved mood stability, improved libido and sexual function, better sleep quality, and improved insulin sensitivity. These are not marketing claims. They are outcomes supported by decades of clinical and research data.

The risks deserve honest disclosure. Testosterone therapy can increase red blood cell count — which is why we monitor hematocrit. It can suppress sperm production — relevant for men concerned about fertility. It can worsen existing prostate conditions — which is why PSA is checked before starting. Fluid retention and acne are possible, particularly early in therapy. These are manageable with appropriate monitoring and dose adjustment. They are not reasons to avoid treatment when the clinical picture supports it. They are reasons to work with a provider who is actually paying attention.

The Bigger Picture

Testosterone is important. It is not the only thing.

The most meaningful results I see in male patients come from addressing the full picture: testosterone when it's indicated, combined with guidance on resistance training and protein intake to support body composition, attention to sleep quality and cortisol burden, evaluation and optimization of thyroid and metabolic function, and the kind of ongoing monitoring that catches adjustments before they become problems.

That is the version of men's health that produces results that last. Not a testosterone prescription and a follow-up in six months. A plan — built around your labs, your symptoms, your goals, and your life.
