The 10 Lab Tests Every Woman Should Ask For — And What They Actually Mean
If you're a woman who has ever walked out of a doctor's appointment with a clean bill of health and still felt completely off — this post is for you.
What I've seen again and again in clinical practice is that the standard annual panel most women receive answers a very limited question: are you acutely ill? It does not answer the questions that actually matter to most women in their 30s, 40s, and 50s. Why am I exhausted? Why is my weight moving in the wrong direction? Why do I feel anxious for no reason? Why has my sleep fallen apart? Why don't I recognize myself anymore?
Those questions have answers. They're just not in the standard panel.
Women's hormones operate as an interconnected system — estrogen, progesterone, testosterone, thyroid, cortisol, and several others are in constant communication with each other and with virtually every other system in the body. When something shifts in one part of that system, it creates a ripple effect. That's why symptoms like cycle irregularity, mood changes, weight gain, brain fog, and fatigue so often arrive together. They're not separate problems. They're connected signals from the same system asking for attention.
Here are the ten labs I consider most essential for understanding what's actually happening hormonally in women — what each one measures, why it matters, and what it can tell us when a standard panel misses it entirely.
1. Estradiol (E2)
Estradiol is the primary form of estrogen in women of reproductive age. It regulates menstrual cycles, supports bone density, stabilizes mood, and plays a direct role in sleep quality and cognitive function.
What most women aren't told: small fluctuations in estradiol — even within the normal range — can trigger migraines, intensify PMS, and disrupt sleep. Irregular estradiol patterns often appear years before menopause as one of the earliest signs of perimenopause. Waiting for estradiol to drop dramatically before investigating symptoms means waiting years longer than necessary.
Timing matters here. For women with regular cycles, estradiol is most informative when drawn on day 3 of the cycle for a baseline reading, or in the mid-luteal phase (around day 21) for a fuller picture of cycle function.
2. Progesterone
Progesterone is the hormone most responsible for the second half of the menstrual cycle — the luteal phase. It supports restful sleep, calms the nervous system through its interaction with GABA receptors in the brain, and is essential for maintaining early pregnancy.
Low progesterone is one of the most commonly missed drivers of PMS, PMDD, short cycles, irregular bleeding, anxiety, and poor sleep. Because it interacts directly with the brain's calming pathways, low progesterone doesn't just affect the reproductive system — it affects how a woman feels in her own nervous system on a daily basis. This is one of the reasons women with low progesterone often feel wired, anxious, and unable to come down at the end of the day.
Like estradiol, timing matters. Progesterone should be drawn in the mid-luteal phase — around day 21 of a 28-day cycle — to accurately reflect peak production.
3. LH and FSH (Luteinizing Hormone and Follicle-Stimulating Hormone)
LH and FSH are the pituitary hormones that signal the ovaries to produce estrogen and release eggs. Together, they tell us how hard the pituitary is working — and whether the ovaries are responding.
In perimenopause and menopause, FSH rises as the pituitary tries to compensate for declining ovarian function. An elevated FSH on a day 3 draw is often one of the earliest objective markers of perimenopause — appearing years before estrogen falls significantly. LH and FSH together also help evaluate ovulatory function, PCOS patterns, and overall reproductive axis health.
4. Testosterone — Total and Free
This is the marker most commonly overlooked in women's hormonal evaluations — and one of the most clinically important.
Women produce and depend on testosterone for energy, lean muscle mass, bone density, cognitive sharpness, motivation, and libido. Testosterone begins declining in women as early as the late 20s, often well before estrogen starts to shift. By the time a woman is in her 40s, her testosterone may already be a fraction of what it was a decade earlier — and nobody has ever measured it.
Like in men, total testosterone alone is not enough. SHBG — discussed below — can bind testosterone and make it unavailable. Free testosterone is the active, usable portion, and it's the number that correlates with how a woman actually feels. A woman with normal total testosterone and high SHBG may be functionally testosterone-deficient even though her total number looks fine on paper.
5. DHEA-S (Dehydroepiandrosterone Sulfate)
DHEA-S is the adrenal precursor hormone — the raw material your body converts into estrogen and testosterone. It peaks in early adulthood and declines steadily with age, stress, and chronic illness.
Low DHEA-S is often the missing piece when a woman's other hormone levels look reasonable but her energy, libido, and recovery are still suboptimal. Because DHEA-S feeds into multiple hormone pathways simultaneously, addressing it can have downstream effects across the entire hormonal picture. It's also a sensitive indicator of adrenal stress — chronically elevated cortisol suppresses DHEA-S production, and the ratio between the two tells a story about how the body is managing its stress burden over time.
6. Cortisol
Cortisol is the body's primary stress hormone — and as I've written about extensively in earlier posts, chronically elevated cortisol disrupts virtually every other hormonal system. In women specifically, cortisol dysregulation suppresses ovulation, throws progesterone production off cycle, contributes to estrogen dominance patterns, and drives abdominal fat storage.
The 2 to 4am waking pattern many women experience — waking in the middle of the night unable to fall back asleep — is frequently driven by cortisol rising too early in the morning as part of a dysregulated circadian rhythm. This isn't a sleep problem. It's a hormonal problem that shows up as a sleep problem.
Cortisol is most informative when measured at multiple points in a day — morning, midday, and evening — to assess the diurnal pattern rather than a single snapshot. A morning blood draw gives a baseline, but it misses the shape of the curve.
7. Full Thyroid Panel — TSH, Free T3, Free T4
Thyroid disorders affect women five to eight times more commonly than men. And yet most women who come in concerned about their thyroid have only ever had their TSH checked.
As I covered in the reading labs post, TSH is a signal from the pituitary — not a direct measure of thyroid output. Free T4 is the storage form. Free T3 is the active form that your cells actually use for energy, metabolism, and temperature regulation. And Reverse T3, which I add when clinical presentation warrants it, is the brake that can suppress T3 activity even when everything else looks normal.
A woman can have a perfectly normal TSH and still have low Free T3, elevated Reverse T3, and symptoms of hypothyroidism that nobody has investigated because the gatekeeper test looked fine. This pattern is particularly common in women under chronic stress or those who have been on prolonged caloric restriction.
8. SHBG (Sex Hormone-Binding Globulin)
SHBG is the protein that binds to sex hormones — primarily testosterone and estrogen — and renders them biologically unavailable. High SHBG means that even if total hormone levels look adequate, much of that hormone is locked up and can't reach receptors.
SHBG rises with high estrogen states, oral contraceptive use, hyperthyroidism, and elevated cortisol. It falls with insulin resistance, low thyroid function, and obesity. Because it sits at the intersection of so many hormonal and metabolic variables, SHBG is one of the most clinically informative markers in a women's hormone panel — and one of the most commonly omitted from standard testing.
When a woman's symptoms don't match her hormone levels, SHBG is often where the answer is hiding.
9. Prolactin
Prolactin is most commonly associated with breastfeeding, but it has broader hormonal significance. Mildly elevated prolactin — even without galactorrhea or obvious symptoms — can suppress ovulation, reduce libido, disrupt cycle regularity, and interfere with fertility. It rises in response to stress, certain medications, and occasionally as an early marker of a pituitary adenoma.
In women presenting with cycle irregularities, fertility challenges, or unexplained low libido who don't have an obvious hormonal driver, prolactin is one of the first markers I add. It's inexpensive to test and frequently overlooked.
10. Vitamin D (25-OH)
Vitamin D appears here because its role in women's hormonal health specifically goes well beyond the general wellness conversation. Vitamin D acts more like a hormone than a vitamin — it influences estrogen metabolism, supports progesterone production, modulates immune function (relevant to autoimmune thyroid conditions, which disproportionately affect women), and plays a role in mood regulation through its effects on serotonin synthesis.
Deficiency is common — more common than most people expect even in Utah, as I've written about before. And in women navigating perimenopause or hormonal imbalance, suboptimal Vitamin D levels quietly undermine outcomes from every other intervention. It is always on my baseline panel.
A Note on Timing and Context
For women with regular menstrual cycles, the timing of hormone testing matters significantly. Day 3 of the cycle (FSH, LH, estradiol baseline) and the mid-luteal phase around day 21 (progesterone, estradiol peak) give the most clinically useful information about cycle function. Markers like thyroid, cortisol, SHBG, DHEA-S, prolactin, and Vitamin D can be drawn at any time and don't require cycle-specific timing.
For women in perimenopause or postmenopause, cycle-specific timing is no longer relevant — but the full panel remains just as important, if not more so.
