Membership terms
Clear expectations.
From the start.
- Minimum commitment 12 months. Early termination is considered only under extraordinary circumstances at Matt's sole discretion. The enrollment fee is non-refundable in all circumstances.
- Direct access hours Available during waking hours, seven days a week. Same-day response is the standard. Major holidays — Thanksgiving, Christmas Day, New Year's Day, July 4th, Memorial Day, and Labor Day — are designated rest days. Non-urgent messages received on these days will be responded to within 24 hours. This program does not replace emergency services. All life-threatening situations call 911 first.
- Vacation & coverage A designated NOVA clinician serves as coverage contact during Matt's absence. Limited telehealth access with Matt is available when accessible. All coverage arrangements are communicated in advance.
- Enrollment fee Non-refundable. Covers the complete onboarding diagnostic workup conducted in the first 90 days of membership.
- Rate guarantee Your monthly rate is locked at enrollment and will never increase for the life of your membership.
- Panel size Maximum 20 members. This is a permanent cap, not a temporary limit. It exists to ensure the standard of care never diminishes.
- Payment Monthly membership billed on the 1st of each month. Enrollment fee due at signing. HSA/FSA eligible.

Matt is a board-certified Family Nurse Practitioner with a Master of Science in Nursing and over a decade of clinical experience. He founded NOVA Wellness with a clear premise: that the standard of care most people receive is a floor, not a ceiling, and that patients who want more shouldn't have to navigate a fragmented, reactive system to get it.
The Executive Program is the fullest expression of that premise — a practice structured around twenty people, designed to be the most thorough and personal medical care available in the state.
